In order to complete this supplemental application, please have the following information available:
- W-2s for all members of the household
- 1099s for all members of the household
- Most current income information for all members of the household
- Bank statements and/or other records of investments
- Records of any untaxed income (examples include Social Security, TANF, Veteran’s benefits, welfare assistance, child support, etc.)
